Medical Records

Complete the authorization form (must be a patient or patient’s guardian/legal representative).

Obtain a copy of your photo ID.

Mail, deliver, or fax the completed authorization form and a copy of the photo ID to:

Once your records are processed, you will be notified of any fees and may then pick up a copy of your medical records in person at the hospital or have them mailed to your home address after payment is received. If you pick up your records in person, you will be required to present your photo ID. You may send a representative to obtain your records only if they are listed on the authorization form under “Release/Obtain the following To/From.” Your representative must provide a copy of both their and your photo ID.

Does LSBH treat children?
No, Lone Star Behavioral Health treats adults 18+.
Does LSBH treat intellectual disabilities or autism?
We do help treat those who have intellectual disabilties and autism, however, our main treatment is mental health based. If you or a loved one needs treatment more catered to intellectual disabilities or autism, we may not be right for you.
Does LSBH treat dementia or alzheimer's?
Unfortunately, we do not specialize in dementia or alzheimer care.
Does LSBH do private practice?
At this time, we do not conduct private practice sessions. We are looking to grow our facility and expand services, stay tuned!
Does LSBH have an evening iop/php program?
We currently do not offer any evening sessions.
What is an iop/php program?
Intensive outpatient programs (IOPs) are treatment programs that do not require round-the-clock supervision. They enable patients to continue with their normal, day-to-day lives in a way that residential treatment programs do not. Whereas residential treatment requires that clients reside on site, clients in intensive outpatient programs live at home.
IOPs are sometimes used in conjunction with inpatient programs as a way of helping clients to more smoothly and seamlessly adapt back into their families and communities. They are designed to establish support mechanisms, help with relapse management, and provide coping strategies.
Is inpatient or outpatient right for me?

Is LSBH a rehab facility?
Lone Star Behavioral Health conducts group and individual sessions processing substance related issues and educates in relapse prevention. Our inpatient facility offers detox, however, we are not a rehab facility.
